Hospice Licensed Vocational Nurse (LVN)

Location: Washington, TX and surrounding area. Position: Full-Time or PRN.

• Provide compassionate nursing care to hospice patients in their homes or care facilities.
• Complete routine patient visits and assist with admissions as assigned.
• Monitor patient conditions and communicate changes to the RN Case Manager and interdisciplinary team.
• Administer medications and treatments as ordered.
• Educate patients and families on disease progression, comfort measures, and medications.
• Maintain accurate, timely documentation using a company-provided tablet.
• Work closely with physicians, RNs, CNAs, Social Workers, Chaplains, and other team members to provide exceptional patient care.
• Active Texas LVN license.
• Hospice, home health, long-term care, or clinical nursing experience preferred.
• Strong communication and organizational skills.
• Compassionate, dependable, and patient-focused.
• Reliable transportation and willingness to travel locally.
• Current CPR certification.
