Responsibilities
- Plan and conduct interviews with individuals and families to gather and evaluate information, assess risk factors, and determine service needs.
- Organize, present, and document case information clearly and accurately to support decisions and recommendations.
- Assess, develop, implement, evaluate, and adjust case plans collaboratively to meet diverse client and family goals.
- Identify and respond to potential safety risks, taking timely action to de-escalate crisis situations.
- Access and coordinate appropriate community resources based on specific client needs.
- Operate a vehicle independently across both rural and urban communities to support service delivery.

Technical Knowledge and Skills
- To qualify for this position, you must possess a strong knowledge of human services-related policies, practices and procedures. Typically, the skills required for this position would be obtained through a diploma or degree in a human services-related field and/or experience working in a human service setting.
