Responsibilities
- Conduct comprehensive medical assessments during initial and follow up home visits.
- Diagnose acute and chronic medical conditions.
- Develop individualized care and treatment plans based on patient needs.
- Prescribe medications in accordance with DOH regulations and evidence based clinical practice.
- Order, review, and interpret laboratory and diagnostic investigations.
- Monitor patients' response to treatment and modify management plans as required.
- Manage patients with chronic illnesses including diabetes, hypertension, cardiovascular diseases, etc.
- Identify medical emergencies and initiate appropriate interventions and referrals.

- Medical degree and DOH license as General Practitioner
- Willing to work in homecare setting

Job Overview
