geriatric assessments

, including functional, cognitive, psychological, and social evaluations.

• Diagnose and manage

complex medical conditions in elderly patients

, including multimorbidity, frailty, and polypharmacy.

• Develop and implement

individualised care plans

focusing on long-term health, rehabilitation, and quality of life.

• Manage

chronic diseases

such as dementia, osteoporosis, cardiovascular disease, diabetes, and neurodegenerative disorders.

• Coordinate care with

multidisciplinary teams

, including physiotherapists, occupational therapists, nurses, and social workers.

• Provide

falls risk assessment, mobility management, and rehabilitation planning
