Responsibilities
- Patient Assessment: Evaluate patients’ mental, emotional, and social needs, including family dynamics, financial situation, and support systems.
- Care Planning: Develop and implement care or discharge plans in coordination with doctors, nurses, and other healthcare staff.
- Counseling & Emotional Support: Provide guidance to patients and families dealing with stress, trauma, chronic illness, or end of life situations.
- Discharge Coordination: Arrange post hospital care such as rehabilitation, home healthcare, or placement in long term care facilities.
