Role Summary

  • Assists patients, families, and significant others with their psychological, social, environmental, financial, and emotional problems by conducting needed psychosocial assessments and providing necessary interventions.

Qualifications

  • Special Certificate: Required
  • Two years of experience

Technical Responsibilities

  • Taking Care Management responsibility for vulnerable clients, including comprehensive assessment, care plan formulation, and resource identification.
  • Carrying out initial comprehensive person centered psychosocial assessments, plan and implement individual strategies of care.
  • Acting as an advocate for the patient in ensuring their expressed wishes are central when planning care.
  • Consider legal issues for patients admitted and liaise with admitting authority such as police, judicial, prosecution.
  • Participate in safe and effective decision making to facilitate a safe, timely discharge.
  • Liaising effectively with other agencies including government and charity.
  • Working closely with patients/family in directing them to appropriate resources.
  • Follow up with psychiatric visits from other hospitals.
  • Respecting the rights of the individual service user and their families.
  • Follow up with approved ER exempted cases of needy patients.
  • Promoting and informing an awareness of social care/social work context.
  • Networking and liaise with other professionals and external organizations.

Additional Responsibilities

  • Provide crisis interventions as needed.
  • Participate in complex discharge planning with the multi disciplinary team.
  • Participate in multidisciplinary team meetings to discuss and advocate for patient and family needs.
  • Engage in providing clinically appropriate psychoeducation and direct therapeutic interventions.
  • Evaluate, assist and counsel children and their caregivers when dealing with issues related to child protection.
  • Contribute effectively to breaking bad news and the bereavement process.

General Proficiency

  • Be familiar with mental health laws and related legislation including Federal law No. (10) of 2023 concerning mental health.
  • Recognize differences in culture, gender, race, ethnicity, religion, language, and socioeconomic status.
  • Adhere to ethical guidelines and safeguard vulnerable groups.
  • Apply effective communication skills, build rapport and be sensitive to patients' individual and social needs.
  • Communicate effectively with colleagues, hold high standards of professional conduct.
  • Continuously reflect on personal biases, limitations and emotional responses and practice self care to prevent burnout.
