- Assist the Social Worker in delivering psychosocial support services and follow up with patients and families
- Conduct initial client interviews or gather demographic/social information for assessment purposes under supervision
- Help patients complete necessary forms, service applications, or paperwork for referrals and resource programs
- Research and coordinate available community resources, support services, and relevant agency contacts
- Maintain case files, update logs, document client interactions, and assist with data entry
- Facilitate communication between patients, families, clinical social workers, and hospital staff
- Provide general emotional support and reassurance to patients and families within the scope permitted
Requirements
- Experience in social work or related field within a healthcare setting
- Ability to conduct initial client interviews and gather demographic/social information for assessment purposes under supervision
- Knowledge of community resources and support services
- Proficiency in maintaining case files, updating logs, and documenting client interactions
- Strong communication and liaison skills to facilitate between patients, families, and healthcare staff
