Job Summary

Job Responsibilities 1

  • Supports hospital policies and serves as professional role model.
  • Contributes to development and execution of Pharmacy department plans.
  • Provides input into development and implementation of pharmacy policies and procedures.
  • Participates in development of new order sets and clinical practice guidelines.
  • Develops clinical pharmacy programs according to policies and regulations.
  • Collaborates with healthcare professionals to ensure optimal patient care.
  • Provides therapeutic recommendations and answers medication related questions.
  • Reviews records to determine appropriateness of medication therapy.
  • Performs chart review and evaluates patient condition.
  • Assesses results of pharmaceutical treatments.

Job Responsibilities 2

  • Evaluates and monitors drug therapies and antibiotic utilization.
  • Develops treatment plans, initiates, titrates/taper, and discontinues medications under protocol.
  • Participates in formulary control and monitoring.
  • Performs Drug Information and Poisoning review and assessment.
  • Gathers and analyzes data for drug utilization reviews.
  • Monitors, reports, and analyzes adverse drug reactions and medication errors.
  • Identifies, resolves, and prevents potential and actual medication related problems.
  • Consults on dosages, medication substances, correct administration.
  • Performs patient education functions.
  • Participates in development and update of drug libraries.
  • Participates in development of hospital wide clinical protocols.
  • Ensures correct practice of handling controlled and narcotic medications.

Job Knowledge & Skills

  • Clinical pharmacy residency Preferred
  • Clinical pharmacist licensure from MOPH in Qatar
  • Strong knowledge of protocols, policies, standards, and local laws.
  • Knowledge of actions, side effects, appropriate use of drugs, antibiotic utilization, evaluation, drug therapeutic monitoring.
  • Knowledge of current technology and pharmacy systems.
  • Knowledge of pharmaceutical and medical terminology.

Job Experience

  • Minimum of 3 years of experience as a clinical pharmacist in a hospital setting.
  • Hands on experience in direct patient care.
  • Minimum 2 years in GCC (preferred).

Education

  • Bachelor's Degree in Pharmacy
