- Supports hospital policies and serves as professional role model.
- Contributes to development and execution of Pharmacy department plans.
- Provides input into development and implementation of pharmacy policies and procedures.
- Participates in development of new order sets and clinical practice guidelines.
- Develops clinical pharmacy programs according to policies and regulations.
- Collaborates with healthcare professionals to ensure optimal patient care.
- Provides therapeutic recommendations and answers medication related questions.
- Reviews records to determine appropriateness of medication therapy.
- Performs chart review and evaluates patient condition.
- Assesses results of pharmaceutical treatments.
Job Responsibilities 2
- Evaluates and monitors drug therapies and antibiotic utilization.
- Develops treatment plans, initiates, titrates/taper, and discontinues medications under protocol.
- Participates in formulary control and monitoring.
- Performs Drug Information and Poisoning review and assessment.
- Gathers and analyzes data for drug utilization reviews.
- Monitors, reports, and analyzes adverse drug reactions and medication errors.
- Identifies, resolves, and prevents potential and actual medication related problems.
- Consults on dosages, medication substances, correct administration.
- Performs patient education functions.
- Participates in development and update of drug libraries.
- Participates in development of hospital wide clinical protocols.
- Ensures correct practice of handling controlled and narcotic medications.
Job Knowledge & Skills
- Clinical pharmacy residency Preferred
- Clinical pharmacist licensure from MOPH in Qatar
- Strong knowledge of protocols, policies, standards, and local laws.
- Knowledge of actions, side effects, appropriate use of drugs, antibiotic utilization, evaluation, drug therapeutic monitoring.
- Knowledge of current technology and pharmacy systems.
- Knowledge of pharmaceutical and medical terminology.
Job Experience
- Minimum of 3 years of experience as a clinical pharmacist in a hospital setting.
- Hands on experience in direct patient care.
- Minimum 2 years in GCC (preferred).
Education
- Bachelor's Degree in Pharmacy
