Overview
Assists patient care in defined units by planning, managing, monitoring and advising specialist pharmaceutical care programs for patients.
Reviews medication orders and patient medication profiles for appropriateness of therapy.
Identifies and undertakes a medication review/ history of newly admitted patients to resolve issues that may influence the patient s treatment plan.
Provides assistance with the treatment plans for inpatients, considering their presenting symptoms, medical history, and the evidence base for pharmacological approaches in that condition.
Conducts clinical evaluations to assess medication use appropriateness.
Creates future treatment options with patients /caregivers after considering and discussing the risks and benefits.
Conducts clinical and cost effectiveness review of medications for addition/deletion from the formulary.
Prepares and oversees medication orders for drug interactions, duplication and omission of therapy, allergies, adverse reactions, and recommend appropriate formulary alternatives as needed.
Advises on pharmaceutical dosage (pediatric doses, IV admixtures, TPN, chemotherapy, etc.).
Liaises with physicians, nurses and other fellow health care professionals to ensure the delivery of safe, effective and economic drug treatment.
Interpret laboratory tests and other diagnostic tools to assess the safety and effectiveness of drug therapy for individual patients, and recommend changes to practice and policy in line with updated evidence based medicine.
Prepares therapeutic recommendations in the patient s medical record and pharmacy intervention forms.
Executes leadership role by supervising and advising unit based pharmacists in the clinical areas.
Contributes to the development of Clinical Practice Guidelines, policies and HIS enhancement that support safe pharmacotherapeutic care.
Facilitates the safe and competent use of all pharmacy equipment used in their area of practice.
Provides regular attendance at multidisciplinary team meetings and medication review sessions making clinical decisions, and is able to support these when challenged with a logical rationale and evidence base.
Participates in department programs/committees, various hospital committees and staff meetings, as assigned.
Participates in the orientation and training of new employees.
Provides drug information and education resource for patients, students, colleagues, medical and nursing staff, and other healthcare professionals.
Maintains professional knowledge by attending and participating in conferences, seminars or online education events.
