CHW Supervisor (Illinois, Chicago)

Supervise and mentor CHW/CHOWs in educating and disseminating health messages in the community, ensuring adherence to protocols and procedures.

Guide CHW/CHOWs in connecting community members to health and social services and navigating case management challenges.

Lead CHW/CHOWs in recruitment, outreach, and providing ongoing care to community members. This includes onsite visits, home visits, and phone consultations with patients, families, and medical/social service staff. May also manage a caseload of participants.

Collaborate with project leaders and other team members to assess intervention effectiveness, monitor and/or assign CHW caseloads, and discuss workloads.

Participate in the hiring process for CHW/CHOWs, conduct regular evaluations for direct reports and help CHW/CHOWs establish professional boundaries.

Foster a positive teamwork environment. This includes recognizing staff achievements and building strong relationships.

Ensure CHW/CHOWs are well-equipped to succeed by managing workloads, documenting their efforts, and overseeing program-specific paperwork (activity logs, case notes, evaluation tools).

Help CHW/CHOWs develop various business skills such as writing, public speaking, record keeping, leadership, and time management. Identifies and plans continuing education opportunities and assists with the facilitation of training (classroom, on-the-job, shadowing, and quality improvement initiatives).

Facilitate effective communication between team members, project staff, community partners, and clients. This may involve working with multi-disciplinary teams in health settings to develop and maintain strong relationships with clinical teams.

Assist with organizing data for program evaluation and ensuring the fidelity of data collected by CHW/CHOWs.

Actively participates in team meetings and/or external partner meetings.
