The Care Coordinator facilitates care coordination for youth enrolled in the Illinois Department of Healthcare and Family Services (HFS) Pathways to Success program. Youth are enrolled in either the High-Fidelity Wraparound Services or Intensive Care Coordination Services. The Care Coordinator will utilize the Principles of Wraparound and System of Care when providing services to families. This position is home-based, meaning there is not an assigned office, but the person is expected to travel within Lake County to attend meetings with youth and their families. Position Duties
Provide direct services to youth and families while managing a caseload of either 10-12 clients for High-Fidelity Wraparound or 16-18 clients for Intensive Care Coordination. Coordinate and monitor delivery of Individual Support Services (ISS) and Therapeutic Support Services (TSS) for youth. Provide referrals to additional Pathways to Success services, such as Therapeutic Mentoring, Intensive Home-Based Services, Family Peer Support, and Respite. Work in collaboration with the youth and family to create a Child and Family Team (CFT) to assist the youth with successfully achieving established goals. Assist youth and families with creating a Crisis Prevention and Safety Plan (CPSP) and update/review as needed. Maintain regular contact with an active caseload, meeting frequency deadlines as determined by the program. Completes required documentation accurately and timely, to include but not limited to, IM+CANS; Strengths, Needs and Cultural Discovery (SNCD), Child and Family Team Meeting (CFTM) agendas, telephone contacts, and progress notes. Maintain IM+CANS certification. Consults with the supervisor regarding status of cases. Facilitates with clients and collaborates with the treatment team to develop, review, and update treatment goals and referrals both internally and externally. Assists clients in obtaining public entitlements, housing, community resources, etc. Attends staffing with Court Services, schools, hospitals, agencies, as necessary. Organizes and participates in treatment-related clinical conferences and staffing’s on assigned clients. Participates in staff meetings, in-service trainings, and seminars to facilitate professional growth and maintain licensure if required. Provides education and direction to clients, family members and/or significant others.

Minimum Qualifications
A bachelor's degree in counseling and guidance, rehabilitation counseling, social work, education, vocational counseling, psychology, pastoral counseling, family therapy, or a related human service field; A bachelor's degree in any other field with two years of documented clinical experience in a mental health setting under the supervision of a QMHP; A certificate of psychiatric rehabilitation from a DHS-approved program, plus a high school diploma or GED, plus 2 years of experience in providing mental health services; A recovery support specialist certified by, and in good standing with, the Illinois Alcohol and Other Drug Abuse Professional Certification Association, Inc.; A family partnership professional certificate from and in good standing with the Illinois Alcohol and Other Drug Abuse Professional Certification Association, Inc.; An occupational therapy assistant licensed under the Illinois Occupational Therapy Practice Act [225 ILCS 75] with at least one year of experience in a mental health setting; A minimum of a high school diploma or GED and 5 years of documented clinical experience in mental health or human services; An individual who has completed a behavioral health technician or other psychiatric training certification through the Medical Education and Training Campus in Fort Sam Houston, Texas, with one year of documented clinical experience in a mental health setting under the supervision of a QMHP. May also qualify if meeting the requirements of a Qualified Mental Health Professional or above. Must possess a valid driver’s license. Requires use of personal vehicle for case management activities, client appointments, crisis response, community outreach or other job-related activities.
