❓ About the Role:
The Alumni Coordinator is responsible for developing, implementing, and sustaining a comprehensive alumni program that supports patients transitioning into long-term recovery.
This role plays a critical part in improving patient engagement, strengthening continuity of care, reducing readmissions, and supporting long-term recovery outcomes.
The Alumni Coordinator serves as a bridge between discharged patients and ongoing recovery support services, ensuring a consistent, patient-centered approach aligned with clinical best practices and regulatory expectations.
This position partners closely with Clinical, Operations, and Business Development teams to build a strong recovery community, reinforce aftercare planning, and support value-based care initiatives.
