Summary: The Wellness Coordinator Assistant will provide quality, respectful, courteous service to all individuals served.

An overview of responsibilities includes but not limited to completing the SAMHSA Unified Performance Reporting Tool (SUPTR), assisting clients with the wellness program activities, and completing administrative tasks.

The Wellness Coordinator Assistant will create a healthy environment for program participants and focus on helping them to improve all areas of their lives while in recovery.

Observe all rules of confidentiality relating to clinical information and treatment within the agency and when communicating with external agencies or individuals.

Assist Wellness Coordinators in conducting the SAMHSA Unified Performance Reporting Tool (SUPTR) at designated times and complete documentation to be entered into a statewide reporting system.

Actively identify and support linkages to community resources that support the clients’ goals and interests.

This will involve a collaborative effort including the client, agency staff, and other relevant community partners.

Assist clients in increasing social support networks of relatives, friends, and/or significant others.
