Social Worker

Conduct comprehensive social work and psychosocial assessments by interviewing patients/residents and families, gathering relevant social information, and identifying factors that may impact coping and care needs.

Maintain accurate patient/resident records, including assessments, progress notes, reports and other documentation in accordance with established standards, policies and regulatory requirements.

Contribute to interdisciplinary discharge planning by assessing care needs, determining appropriate services and collaborating with hospital teams, community partners and other resources for patients/residents with complex needs.

Provide direct and consultative support to patients/residents, families, staff and community health professionals regarding community resources, financial assistance and access to appropriate services.

Participate in interdisciplinary care conferences related to patient/resident reviews, admissions, care planning and discharges.

Support patients, families and care teams in understanding relevant legislative processes, including substitute decision-making, Medical Assistance in Dying, Mental Health Act rights, incapability assessments and adult guardianship.

Develop and deliver education, in-services, presentations and training for staff in collaboration with members of the interdisciplinary team.

Facilitate and participate in group meetings, provide clinical counselling and deliver therapeutic and psychosocial education as required.

Participate in program and practice evaluation, committees, research and quality improvement initiatives to enhance patient/resident care and service delivery.

Maintain current knowledge of developments in Social Work practice and provide teaching, guidance and supervision to social work students as assigned.
