We are seeking a talented and motivated frontend engineer to join our team and play a key role in helping build our patient mobile app and physician facing front ends.
This is a fully remote position.
You are an experienced backend engineer who enjoys owning complex technical problems from discovery through production support. You are highly proficient in modern Python, care about reliability and data integrity, and are comfortable working with systems where safety and accuracy matter.
You communicate clearly, document important decisions, and work effectively in a distributed, async-first environment. You are interested in healthcare and motivated by the opportunity to improve how patients, clinicians, and care teams interact with technology.
Build and maintain integrations between UpDoc and third-party healthcare services, including electronic health record (EHR) systems.
Own integration projects end to end, including technical discovery, scoping, development, testing, deployment, monitoring, and ongoing maintenance.
Design and implement reliable internal and external APIs using modern, type-hinted Python.
Develop backend services using FastAPI and Pydantic.
Work with healthcare interoperability standards such as FHIR, HL7, and SMART on FHIR as integration requirements evolve.
Diagnose production integration issues by inspecting API payloads, healthcare data resources, application logs, and system traces.
Resolve authentication, data-mapping, data-quality, and interoperability problems.

Design integrations that account for network failures, duplicate events, retries, incomplete source data, and other real-world edge cases.
Work with hospital IT teams, implementation partners, healthcare vendors, and internal stakeholders to support new deployments.
Contribute to UpDoc’s patient-facing conversational agent and the backend services supporting clinical AI workflows.
Help build scalable, low-latency systems for conversational, voice, and multimodal experiences.
Collaborate with clinical experts to ensure that UpDoc’s services are accurate, reliable, safe, and trustworthy.
Write comprehensive unit, integration, and end-to-end tests.
Contribute to DevOps, infrastructure-as-code, CI/CD, containerization, monitoring, and observability.
Participate in code reviews, architecture discussions, technical planning, and production troubleshooting.
Document integration architecture, implementation decisions, operational procedures, and reusable patterns.
Support UpDoc’s HIPAA, SOC 2, privacy, security, and healthcare compliance requirements.
At least 5 years of professional software development experience, including significant backend development experience.
Strong professional experience with modern, type-hinted Python.
Experience developing production APIs with FastAPI, Pydantic, or comparable Python technologies.
Experience designing and implementing RESTful APIs.
A strong understanding of backend engineering fundamentals, including HTTP, networking, caching, database design, authentication, and distributed systems.
Experience integrating with third-party APIs and services.
Extensive knowledge of software testing methodologies and practices.
Experience troubleshooting complex technical problems in production environments.
Experience with cloud platforms, particularly Azure.
Experience with containerized applications.
Familiarity with DevOps, infrastructure-as-code, CI/CD, and observability practices.
The ability to build reliable, maintainable systems in an environment where safety matters.
Strong written communication skills and comfort working in a distributed, async-first team.
A demonstrated ability to learn continuously, work independently, and own projects end to end.
An interest in improving healthcare through safe and thoughtfully designed technology.
Experience integrating with Epic, or other major EHR platforms.
Hands-on knowledge of FHIR, HL7 v2, SMART on FHIR, or related healthcare interoperability standards.
Experience designing secure systems that handle protected health information.
Familiarity with HIPAA and SOC 2 requirements in a software environment.
Professional experience building end-to-end LLM-powered applications.
Experience integrating or optimizing language models in production.
Knowledge of LLM evaluation frameworks and MLOps or LLMOps practices.
Experience building AI infrastructure from zero to one.
Experience developing low-latency voice or multimodal applications.
Experience with model fine-tuning, alignment, prompt engineering, or AI safety systems.
Experience in an early-stage startup.
Experience working closely with clinicians or other healthcare subject-matter experts.
