The Registered Nurse (RN) is responsible for ensuring accurate, timely, and compliant clinical documentation for adult home care services. This role includes completion and review of assessments, renewal assessments, care plan development and review, supervisory visits, and oversight of care delivery documentation.
The RN plays a critical role in maintaining compliance with state regulations, Medicaid requirements, and PrimeCare Home Care Services’ policies and procedures. Working independently with minimal supervision from clinical leadership, the RN ensures clinical accuracy, patient safety, and regulatory readiness across all assigned caseloads.

Complete and review initial assessments and renewal assessments in accordance with regulatory timelines.
Review and update individualized plans of care to ensure alignment with assessed needs and physician orders.
Review care delivery documentation for accuracy, completeness, and compliance.

Adult home care or home health nursing experience preferred.
Experience completing and reviewing clinical assessments and care plans.
Supervisory visit experience preferred.
Working knowledge of Medicaid and state home care regulations preferred.
Ability to read, analyze, and interpret clinical documentation, regulations, and policies.
Strong written and verbal communication skills.
Ability to provide clear, professional feedback related to documentation and care delivery.

Adheres to all PrimeCare Home Care Services policies, procedures, and ethical standards.
Demonstrates commitment to continuous quality improvement.
Maintains confidentiality of patient, employee, and organizational information.
Meets attendance, punctuality, productivity, and safety requirements.
Frequent standing, walking, sitting, and use of hands.
Occasional bending, kneeling, or crouching.
Ability to lift up to 30 pounds using proper body mechanics.
Frequent use of computers and electronic documentation systems.
Combination of office-based and in-home work settings.
Moderate noise levels and potential interruptions.
May require meeting strict regulatory and documentation deadlines.
