In this role, the RN Clinical Consultant partners closely with Lead Care Managers (LCMs) and interdisciplinary team members to ensure care delivery is clinically appropriate, compliant, and aligned with healthcare plan requirements.
The Clinical Consultant supports clinical decision-making, risk stratification, and care planning across the ECM population.
The RN Clinical Consultant is responsible for oversight of Comprehensive Assessments, Care Management Plans, Transitions of Care, and clinical escalations, ensuring timely interventions and continuity of care.
This role plays a critical part in improving health outcomes, reducing avoidable utilization, and supporting members through complex care journeys.
ECM services are provided to eligible Medi-Cal members enrolled in managed care plans and focus on sustained engagement, care coordination, and connection to community-based resources.
Through collaboration with internal teams and external providers, the RN Clinical Consultant helps ensure members receive the right care, at the right time, in the most appropriate setting.

This position is remote and is based out of California.
Standard business hours are Monday through Friday from 8:00 am to 5:00 pm.
