Minimum of 5 years as a Registered Nurse
Active RN license: compact, multi-state licensure
Strong background in care management as well as complex and chronic condition management
Skilled in motivational interviewing, patient education, and patient advocacy
Ability to work independently in a remote environment while staying connected to the team
Comfort with technology and multiple EMR systems
Bachelor’s Degree (Registered Nurse)
Specialty Certification in Care Management (CCM)
Multilingual a plus
The salary range for this role is between $78,000.00 to $83,000.00 per year.
