Essential Experience

12 months experience supporting adults within a healthcare, rehabilitation or neurological setting

12 months experience working with individuals with complex physical disabilities or neurological conditions.

Understanding of person-centred care and rehabilitation principles.

Essential Qualifications

  • NVQ/QCF Level 2 or Level 3 in Health and Social Care (or equivalent).
  • GCSEs (or equivalent) in English and Mathematics
  • Deliver rehabilitation programmes prescribed by Physiotherapists, Occupational Therapists and Speech & Language Therapists.
  • Support residents to achieve rehabilitation goals through mobility, exercise, balance and functional activity programmes.
  • Assist residents with activities of daily living, promoting independence, confidence and self-care skills.
  • Support the use of mobility aids, prosthetics, specialist equipment and assistive technology.
  • Provide rehabilitation support to residents living with neurological conditions, acquired brain injuries, stroke-related disabilities and other complex conditions.
  • Monitor, observe and report changes in residents' physical, cognitive, emotional or behavioural wellbeing.
  • Support residents experiencing communication difficulties, cognitive impairments, fatigue, sensory deficits or behavioural challenges.
  • Build positive therapeutic relationships with residents and their families.
  • Motivate and encourage residents to actively participate in rehabilitation and recovery programmes.
  • Promote dignity, choice, privacy, independence and person-centred care at all times.
  • Support residents to access social, leisure, educational and community opportunities where appropriate.
  • Maintain accurate records of interventions, participation, progress and outcomes.
  • Work collaboratively with therapists, nurses and the wider multidisciplinary team, contributing to handovers, MDT meetings and care reviews.
  • Assist with rehabilitation assessments and outcome measures as directed by qualified clinicians.
  • Follow all safeguarding, infection prevention, health and safety, moving and handling and risk management procedures.
  • Maintain a safe, clean and therapeutic environment that supports resident wellbeing and recovery.
