Registered Practical Nurse - Visiting Nursing

As a Registered Practical Nurse, you’ll provide quality and compassionate care to individuals in their homes. Our team of Nurses are dedicated to making a difference where it matters most: at home. You'll use specialized skills to enhance the daily lives of clients, providing the specialized care that only nurses can offer. In this role, you aren't just a clinician-you are a trusted partner to families and a key collaborator in the healthcare journey to ensure safe, effective and meaningful care.

Complete program, clinical or organizational documentation in a timely and accurate manner

Participate in organizational learning opportunities as appropriate (e.g, Surge Learning)

Engage in organizational working groups as appropriate (e.g, JH&SC, Quality Committee)

Effectively utilize organizational tools, software, devices in the application of daily work, willingness and ability to learn new technical skills as operational or patient needs change

Willingness and ability to provide care in a variety of geographical locations (eg. Central Hamilton), patient care environments (eg. apartment building), or institutional care settings (eg. Long Term Care)

Willingness and ability to act as coach/mentor/Preceptor to new nurses through knowledge sharing and peer-to-peer support

Diploma in Practical Nursing from an accredited college

Registered with the College of Nurses of Ontario (CNO)

Current BCLS/HCP certification

1–3 years of clinical experience (required)

Experience with IV medication administration (required)

Home and community care experience (preferred)

Familiarity with mobile work tools and EMR systems
