Community Care Cooperative (C3) is a 501(c)(3) non-profit multi-service organization governed by Federally Qualified Health Centers (FQHCs). Our mission is to leverage the collective strengths of FQHCs to improve the health and wellness of the people we serve. We fulfill two primary business functions for the FQHCs we work with. These services include Epic EHR licensing and implementation; pharmacy operations consulting as well as the building and direct operation of on-site retail pharmacies; billing and credentialing; employer-sponsored insurance; and others.

This position provides support to the organization’s Quality Program, including, but not limited to: supporting the development, management, and implementation of the annual quality plan; monitoring quality performance on value-based contracts; providing expertise on quality management and improvement; identifying quality improvement opportunities; working cross-functionally with health centers; supporting and C3 health center facing teams to facilitate improvement, and managing various administrative and regulatory requirements of the quality program. This position reports to the Director of Quality.
