DUTIES AND RESPONSIBILITES:
- Ensure that the Community Residential Alternative (CRA) homes’ needs are being met by the supervisor’s duties and responsibilities such as and not limited to grocery shopping, food menu, home maintenance and staffing.
- Monitor program services and changes as they occur.
- Report and document Individual and staff incidents according to policy and procedures and regulations.
- Home visits/ DDP Reports required for Individuals in their private residence in the Community Living Support (CLS) program.
- Ensure the timely filing and retrieval of documents in the CLS homes – making sure that the Case Manager/Support Coordinator, staff and the Individuals are able to access the documents.
- Transport Individual(s) to community outings and/or scheduled appointment(s) in a company vehicle as needed.
- Assist with Individual Service Plan, ISP meeting and provide feedback as needed.
- Oversee Direct Support Professional’s documentation via paperwork, files (filing) as well as electronically.
- Maintain Individuals’ safety.
- Oversee emergency/disaster preparedness policy, plans and confirm emergency drills are conducted as scheduled in licensed and non-licensed homes.
- Develop emergency/disaster preparedness plans with Direct Support Professional and family for the Individual in a non-licensed home.
- Maintain compliance with all Individual charts: auditing, filing, and reducing to store in archives.
- Assist with scheduling for Individuals and Direct Support Professionals as needed and required. OTHER DUTIES AS ASSIGNED:
- Filing, telephone communication, documenting in the call center, faxing, shredding, and correspond with emails daily.
