Position Summary

  • The Patient Care Coordinator is responsible for front office operations including patient scheduling, insurance verification, authorizations, registration, and communication with referring providers.
  • The ideal candidate is dependable, detail oriented, and enjoys helping patients navigate their healthcare journey.
  • This position is based primarily in our St. Augustine office, with occasional coverage and support in our Jacksonville location.

Patient Experience & Front Office Coordination

  • Welcome patients and provide a positive, professional first impression
  • Answer incoming phone calls and assist patients with scheduling and questions
  • Schedule consultations, procedures, ultrasounds, and follow up appointments
  • Confirm appointments and maintain provider schedules
  • Coordinate patient transportation when needed
  • Collect and update patient demographics, insurance information, and required forms
  • Ensure patient charts are prepared and complete prior to appointments

Insurance & Administrative Support

  • Verify insurance benefits and eligibility
  • Obtain prior authorizations and referrals as required
  • Communicate with insurance companies regarding coverage and approvals
  • Assist with claim follow up, denials, and appeals as needed
  • Maintain accurate documentation within the EMR and practice management systems
  • Scan and upload records, reports, and outside documentation

Provider & Referral Coordination

  • Communicate with referring physician offices and facilities
  • Obtain medical records, imaging, and laboratory results
  • Coordinate pre procedure requirements and patient instructions
  • Ensure continuity of care between patients, providers, and referral sources

Qualifications

  • Minimum 2 years of medical office experience preferred
  • Experience with insurance verification and authorizations strongly preferred
  • Excellent customer service and communication skills
  • Strong organizational and multitasking abilities
  • Ability to work independently and as part of a team
  • Proficiency with computers, EMRs, Microsoft Office, and general office software
  • Reliable transportation and willingness to travel occasionally between St. Augustine and Jacksonville
