Requirements
- Doctor of Dental Surgery (DDS) or Doctor of Medicine in Dentistry (DMD) from an accredited dental school
- Successful completion of an accredited Oral and Maxillofacial Surgery residency program
- Board certification in Oral and Maxillofacial Surgery and valid state licensure
- A minimum of 5 years clinical experience in oral maxillofacial surgical practice
