Position #: 163064 Expected Start Date: To be decided; open until filled Union: SUN Facility: Craik and District Health Centre City/Town: Craik Department: Primary Care Sites Type: Full-time regular FTE: 1 Shift Information: Days, Evenings Number of Hours per Rotation: 28 shifts of 8 hours per 6 weeks Relief: No Float: No Hours of Work: Standard Hours Salary or Pay Band: Pay Band Nurse Practitioner $52.590 to $62.910 Travel Required: Yes Job Description: Reporting to the Director, Primary Health Care Teams and in collaboration with physicians and health care provider team members, the Registered Nurse (Nurse Practitioner) will provide Primary Health Care (PHC) services to assigned populations which may include: urban populations, visiting services to rural communities, and/or priority populations (e.g. newcomers, teens) as determined by community and program needs. Depending on community and team requirements, the position may include first on-call support. As the PHC Team evolves and based on needs of the population, the position may be required to provide new practices such as nurse led extended hour clinic services and/or first on-call support. Primary Health Care services include: health assessment, health promotion and education, disease and injury prevention and assessment, treatment interventions; collaboration with, and referral to, other health care providers; participation in Lean, patient safety, community development, clinical practice redesign, quality improvement, and evaluation. Human Resources Exemption: No Experience

  • Minimum of 3 years' experience working as a RN in PHC setting preferred

Education

  • Master of Nursing - Nurse Practitioner (MN-NP)

Licenses

  • Valid Class 5 driver's license

Other Education & Training

  • ACLS training required upon hire
  • Basic Cardiac Life Support Level C Certificate (BCLS)

  • Knowledge in clinical evaluation processes
  • Knowledge in evidence based models of treatment intervention and application
  • Ability to use various clinical/health assessment tools

  • Knowledge in applicable health promotion and health prevention initiatives
  • Knowledge of quality improvement initiatives
