Position Summary
Volare Health is seeking an experienced and detail-oriented Licensed Practical Nurse (LPN) to join our team as an MDS Nurse .
The MDS Nurse works closely with the interdisciplinary team to assess resident needs, coordinate the assessment process, and ensure accurate and timely completion of required assessments in accordance with state and federal regulations.
Key Responsibilities
- Participate in the pre-admission process and obtain essential clinical information from referral sources to support accurate MDS planning.
- Collaborate with the interdisciplinary team to review resident care and determine the most appropriate Assessment Reference Date (ARD).
- Ensure accurate, timely, and compliant completion of resident assessments in accordance with state and federal regulations.
- Complete accurate MDS coding using information gathered through resident and family interviews, staff communication, direct observation, and medical record review.
