Job Title: Home Health Care | Licensed Practical Nurse (LPN) Part-Time

We are seeking compassionate and dedicated Licensed Practical Nurses (LPNs) to join our team.

In this role, you will provide skilled nursing services and conduct initial assessments for new clients in Polk County.

You will ensure patients receive the highest quality care in the comfort of their homes.

• Initial Assessments: Conduct assessments for new clients to determine their unique care needs and develop individualized care plans.
• Skilled Nursing Care: Administer skilled nursing activities and procedures in accordance with care plans and best practices.
• Home Visits: Perform home visits to monitor patient progress and provide ongoing care.
• Customer Feedback: Administer customer service satisfaction surveys on a quarterly basis to gather feedback and improve patient experiences.
• Addressing Concerns: Promptly resolve any grievances or complaints received from patients to ensure their comfort and well-being.
• Team Supervision: Provide guidance and supervision to CNAs and HHAs, ensuring high-quality care is delivered.

• Licensure: Current and valid LPN license in good standing.
