HOME CARE NURSE / CARE GIVER

Overview

• Administer medications and treatments as prescribed by physicians, meticulously documenting dosages and times to ensure patient safety and adherence to the care plan.
• Monitor vital signs (blood pressure, pulse, temperature, respiration) regularly and report any significant changes to the supervising nurse or physician promptly.
• Assist patients with activities of daily living (ADLs), including bathing, dressing, grooming, and toileting, while promoting their independence and dignity.
• Prepare nutritious meals and assist with feeding, considering dietary restrictions and preferences to maintain patients' health and well-being.
• Provide companionship and emotional support, engaging in meaningful conversations and activities to combat loneliness and improve the patient's quality of life.
• Observe and document any changes in the patient's physical or mental condition, reporting concerns to the appropriate healthcare professionals for timely intervention.
• Maintain a clean and safe environment for the patient, including light housekeeping duties and infection control measures to prevent the spread of illness.
• Assist with mobility and transfers, using proper techniques and equipment to prevent falls and injuries, ensuring patient safety.
• Provide wound care, including dressing changes and monitoring for signs of infection, following established protocols to promote healing.
• Accompany patients to medical appointments and assist with errands, providing support and advocacy to ensure their needs are met.
