As an Outreach Support Mentor, you will provide one-on-one support to individuals within our Supported Independent Living, Outreach, and Respite programs, both in their homes and in the community.

During your shift, you may support multiple individuals throughout the day, helping them build independence, develop life skills, access community resources, attend appointments, and participate in meaningful activities.
Although you work one-on-one with the individuals you support, you are not alone.
Outreach Support Mentors collaborate closely with other team members to ensure the needs outlined in support plans are met and to empower individuals to achieve their personal goals.
You will help each person feel safe, respected, empowered, and included while supporting them to build independence, make meaningful choices, and participate in their communities.
Success in this role means providing consistent, person-centred support that reflects each individual's goals, strengths, preferences, and needs while helping them develop the skills and confidence to live as independently as possible.

Hours of Work
As we provide 24/7 support, a variety of shifts may become available based on the operational needs of the individuals and programs we support.
Future Outreach Support Mentor opportunities may include shifts between 10:00 a.m. and 8:00 p.m., Monday through Sunday .
Schedules will vary based on program and individual needs.

Applicants should be comfortable commuting to communities throughout the Interlake-Selkirk Region.
