PHP/FPDC is an intermediate care facility for the intellectually and developmentally disabled. We care for individuals 24/7 in a set of four (4) homes that hold up to 16 individuals. We also have a Day Program the clients attend Monday - Friday.

Job Title: Day Program Instructor (Full-Time/Non Exempt) Location: Wings Day Program Schedule: Monday–Friday, 7 AM–3 PM
We are seeking one full-time Day Program Instructor to join our team. In this role, you will assist in implementing person-centered supports for individuals in our Day Services Program. You will help with daily activities, personal care, and community involvement, ensuring that quality care is provided in line with Federal, State, and local standards.
Position Summary This position will serve as an integral member of the Day Services Team and will assist with the day-to-day activities of the program. Assist in the implementation of person centered supports in the habilitation process including personal care, daily activities and community-based activities focusing on the participants choices. Ensures that quality of care, services and training are provided to individuals in accordance with Federal, State, and local standards.
