There are four (4) positions available for this role. The PATH (Patient Access to Health) Care Coordinator serves as the primary point of contact and front door to the Care Management department, providing high-touch customer service, patient navigation, referral coordination, and operational support to patients, caregivers, providers, health plans, and office staff. This role assists patients in understanding health plan benefits, accessing healthcare services, scheduling appointments, obtaining referrals, and connecting with internal and community-based resources. The PATH Care Coordinator serves as a communication liaison between patients and care teams, helping reduce barriers to care and improve the overall patient experience. The PATH Care Coordinator performs non-clinical care coordination activities, supports referral intake and routing processes, assists with population health initiatives, and identifies opportunities to connect patients with appropriate care management services. Clinical, behavioral health, social, or psychosocial concerns are escalated to licensed nurses, social workers, or other appropriate clinical staff. In addition to patient-facing responsibilities, the PATH Care Coordinator provides administrative and operational support to ensure efficient workflows, communication, referral management, and continuity of care across Care Management programs and services. SPECIFIC SKILLS NEEDED · High level of communication skills demonstrating courtesy and professionalism. · Excellent organizational skills, problem solving, and decision-making abilities. · Knowledge of healthcare terminology, insurance benefits, referrals, and authorization processes. · Strong attention to detail and ability to manage multiple tasks in a fast-paced environment. · Proficient in Windows, Microsoft Word, and Excel. · Minimum typing speed of 40 wpm. · Knowledge of medical terminology. EDUCATION/EXPERIENCE/TRAINING Required: · High school diploma or GED · Minimum of two years of experience in health care field Preferred: · Associate's degree or Bachelor's degree in Healthcare Administration, Public Health, Social Services, or related field · Experience in customer service, case management, managed care, or healthcare coordination · Spanish Speaking
