Conduct comprehensive psychosocial assessments and provide clinical interventions for children, adolescents, and families living with new or existing diagnoses of Type 1 Diabetes.
Implement clinically sensitive and evidence-informed clinical interventions to promote family engagement in care to and to improve health and psychosocial outcomes.
Facilitate access to appropriate community resources and support services while collaborating with the broader diabetes care team to address patients’ and families’ psychosocial needs.
Participate in STRiDE leadership meetings to support program evaluation, quality improvement initiatives, and ongoing program development.
Participate in STRiDE clinical case conferences to collaborate with participants’ multidisciplinary care teams and develop comprehensive, coordinated plans of care.
Collaborate closely with the Endocrine Mental Health Team to provide endocrine-related behavioral health support, consultation, and clinical coverage.
Social work experience in medical environment and with child protection experience preferred.
Flexibility and ability to work in fast-paced, dynamic environment.
Excellent teamwork and communication skills necessary.
Spanish-speaking preferred but not required.
LCSW Social Work licensure in the state of Massachusetts, LICSW preferred
