Case Manager - MSTS

Austin Mental Health Division (MHD) provides care and services through a comprehensive range of teams to meet the needs of mental health consumers and carers throughout Victoria. Services in the Mental Health Division are located across Austin Health and the community. Employees may be redeployed within the MHD. All mental health services work within a clinical framework that promotes recovery-oriented practice and supports decision-making. This approach to client wellbeing builds on the strengths of the individual working in partnership with their treating team. It encompasses the principles of self-determination and individualised treatment and care.

As a Case Manager, you will work collaboratively with the MSTS Consultant Psychiatrist and the team to provide comprehensive case management services to individuals experiencing mental health challenges. This role ensures that consumer receive coordinated, person-centered care aimed at improving their psychological, emotional, and social well-being. The service operates between 8.00 am and 9.30 pm Monday to Friday and 8.30 am to 5.00 pm on weekends and public holidays. Case managers work on shifts over the course of the week.

A regular day will see you:

Undertaking assessment and case management of NEMSTS consumers and contribute to the implementation, monitoring and evaluation of treatment objectives

Deliver high standard quality care for a clinical caseload, as well as consultation and liaison with family and organisations

Providing assertive mobile outreach for consumers in their local community

Medication monitoring for consumers in the community setting

Together with the Consultant Psychiatrist and Registrar and the consumers planning clinical interventions that are appropriate for consumer and making sure implementation is done to the high standards

Providing knowledge and skills, based on professional background, as part of a multidisciplinary team, consult with other mental health staff on specialist and community psychiatry matters

You will be someone who has excellent written and verbal communication skills, and the ability to work both autonomously and as a part of the team. You’ll bring energy and drive to your work, and have a keen interest in developing your skillset.

Full Victorian Driving License

Relevant professional qualification in a health-related discipline (Social Work, Occupational Therapy, Clinical Psychology or Registered Psychiatric Nurse) and current registration with the relevant professional board, association or college
