Your Role as Case Manager – Lockyer Valley Housing Service
- The Case Manager, Lockyer Valley Housing Service (LVHS), supports individuals and families experiencing or at risk of homelessness through assessment, case management, advocacy, service coordination and referrals. Working across centre-based, outreach and remote service delivery, this role helps clients secure and sustain stable housing while fostering independence and connections to community supports.
- Key responsibilities of this role include:
- Assessing eligibility, needs and risks of individuals and families experiencing or at risk of homelessness.
- Providing holistic case management, including support planning, advocacy and tenancy sustainment assistance.
- Coordinating services and referrals to ensure clients can access the support they need.
- Build and maintain effective relationships with community partners, service providers and stakeholders.
- Delivering a high-quality, customer-focused service that places client needs at the centre of all interactions.
- Maintaining accurate case notes, client records, reports and program data in line with organisational requirements.
- Working collaboratively within a multidisciplinary team and participate in meetings, supervision and professional development.
- Supporting a safe, inclusive and values-driven workplace, including mentoring volunteers and student placements when required.
