As a Closing the Gap Care Coordinator, you will work alongside Aboriginal and Torres Strait Islander people living with chronic health conditions, helping them access the care, services and supports they need.
You'll coordinate personalised care plans, connect clients with health and community services, and work closely with GPs, Aboriginal health services and other providers to deliver culturally safe, person-centred care.
Aboriginal and Torres Strait Islander peoples are strongly encouraged to apply.
