Position Overview
The Care Coordination Data Entry Specialist works independently assisting the Operations Teams with delegated tasks as needed such as enrollment side duties, outreach to members, mailing documents, and data entry into state systems. This position will be supporting members from different health plans and products.
Schedule
- Full time position, day shift hours, no evenings, weekends or holidays. Hours are 8am to 5pm Monday thru Thursday & 8am to 3pm on Fridays.
Location
- Include description of radius as necessary
Salary
- $24.00 - $25.00 per hour. Salary will be commensurate with experience.
Responsibilities
- Assist with mailing out time sensitive documents to members
- Complete outreach calls to members and or legal guardians to follow up on necessary documentation needed for support plans
- Work efficiently and accurately in the MnChoices system
- Looking up members in MNITs for eligibility of waivers and insurance information
