Horizon Health has an opportunity for a float Advance Practice Provider to cover Family Medicine with the potential to work in other specialty areas in the future.

The Advance Practice Provider (APP) will initially provide primary care for our patients in outlying clinics. We are looking for an APP candidate with experience, compassion, and strong clinical skills. This APP will assess patients regarding their conditions, order and interpret diagnostic tests, and work with patients and families to establish care plans.
Current license as an Advanced Practice Registered Nurse in the State of Illinois. Willing to get Indiana licensure.
Cardio-Pulmonary Resuscitation-BLS.
Preferred three (3) years of Advance Practice Provider experience.

Flexible; willing to work at any of our locations if needed; Marshall, Paris, Tilton, Chrisman, Oakland locations in Illinois and Terre Haute in Indiana.
